Please come into one of our offices and fill out a portal authorization request form.  We apologize for the inconvenience but this must be done in the office.

If you have already filled out a portal authorization request form and:

Need to finish registering for the portal

Option #1: Use the link provided in the email our office sent shortly after you signed up for the portal.  This is the easiest way to finalize your submission

Option #2: Click here to create to finish your registration

If you have already finished registering for the portal, use the "Log in" box on the right to log in or reset your forgotten password.
